Proof of Residency

Please submit a new Proof of Residency dated AFTER July 1, 2023. Please do this even if you submitted proof of residency with your application. We are required by the state to get a new POR dated after July 1 every year.

If you do not want to submit a POR electronically, please bring a copy when you come to get your student’s schedule. Bringing a copy that you can leave with us will shorten the wait time.

Legion Collegiate Academy seeks your consent in allowing us to publish photos that may involve your child’s image on our website, social media pages, news bulletins, and our school yearbook.

Student Parking Pass

Please complete this form ONLY if your student needs a parking pass for this school year.

Payment for the parking pass can be made online at our website HERE. 

Or, if you prefer to pay in person, we can only accept cash or check at the school.

Military Opt Out Form

Please complete and sign below ONLY if you, or your student (if they are 18 years of age or older), would like the student's name, address, telephone listing and/or other directory information withheld for military recruitment purposes.

If you, or your student (if they are 18 years of age or older), would like your student's information released to a military recruiter, you do NOT have to complete this form.
